SCHEDULE YOUR TEAM'S TRAINING

Please be prepared to provide the following:

• Number of employees who need certification
   
• Preferred training dates and location (on-site or at our facility)
   
• Course types needed (e.g., Adult, Pediatric, First Aid, BLS)
   
• Payment preference and deposit (required for 6+ participants)
  

Once submitted, our team will review your request and contact you within 1–2 business days to finalize scheduling and payment details.

 A $150 non-refundable deposit is required to reserve your training session for 6 or more participants. This amount will be applied to your final invoice. 

• You are an authorized representative of your organization.
   
• You agree to our Client Service Agreement at the end of this form
